Registration Form

Fees are based on a 36-week season. Ten post-dated cheques are required when registering. Please date each cheque the first of every month from September 1 to June 1. A $15.00 non-refundable registration and processing fee if pre-registered in May, and a $20.00 after May must accompany registration form. There is a $30.00 costume deposit due Sept. 1, 2008 Fee Schedule

WAIVER I hereby authorize the staff of Burlington School of Classical Ballet and its associates to act for me according to their best judgment in any emergency requiring medical attention, and I hereby waive and release Burlington School of Classical Ballet and its associates from any and all liabilities for any injuries or illnesses incurred while attending classes at the school, on the school premises, or during out-of-school performances.
